Helping you to be happier, healthier and at home

We work with local people and our partners to continuously develop and improve the services provided to people across North East Hampshire and Farnham.

In recent years we have been focusing a lot of effort on supporting people to be happier and healthier in their own homes, by moving more services into local communities and helping the clinicians and health professionals to work more closely together as joined-up teams. This means that people are more able to manage their own health and that if they do need to go into hospital we can support them so that they spend less time in hospital and are back home recovering at home or in their community much more quickly than in the past.

Our work with our partners across the Frimley Health and Care Integrated Care System, as in the video below, is a good example of this new way of working.

What is a CCG?

A clinical commissioning group is an NHS organisation set up by the Health and Social Care Act 2012 to organise the delivery of NHS services in England.  It is clinically led by GPs and clinical staff who work with patients and are best placed to make decisions about local healthcare commissioning.   Find out more about the CCG

Improving health and care services in the Fleet locality

Health and care organisations are coming together to improve the services offered to local people in Fleet, Church Crookham, and Crondall. The four GP Practices which cover the area are helping to plan for increases in the number of people living in the local area, as well as the changing demands of people who are living for longer and often with more complex needs.
