Helping you to be happier, healthier and at home

In recent years we have been focusing on supporting people to be happier and healthier in their own homes, by moving more services into local communities and helping clinicians and health professionals to work more closely together to provide a joined-up service for local people. We are also working to support people to better manage their own health. If people do need to go into hospital we can support them to have a shorter stay so that they are back recovering at home or in their community much more quickly than in the past.

It's very important to us that our work reflects local people's needs and preferences. We know from experience that when local people are involved in designing services that the outcomes are better. When you see this symbol (left) anywhere on our site it means that the services/documents etc., being described on that page have had patient or public involvement in creating them or that the work is in response to input from local people.

What is a CCG?

A clinical commissioning group is an NHS organisation set up by the Health and Social Care Act 2012 to organise the delivery of NHS services in England.  It is clinically led by GPs and clinical staff who work with patients and are best placed to make decisions about local healthcare commissioning.   Find out more about the CCG

Update on the temporary closure for cleaning of The Ferns Medical Practice, Farnham

The Ferns Medical Practice in Farnham Hospital was temporarily closed on 13 February to enable the surgery to be cleaned as a precautionary measure, after recognising that a patient was seen at the practice premises on 12 February, who had travelled from one of the affected Coronavirus areas.

The Ferns Medical Practice in Farnham Hospital is temporarily closed today (13 February 2020) to enable a clean of the surgery as a precautionary measure. The practice will reopen as soon as possible and patients with existing appointments today will be advised if their appointment needs to be rearranged.
